Disaster Prep Enrollment Form – use to enroll chapter members in City of Berkeley disaster preparedness courses.

Service Tracking Form – use to record philanthropy and community service activities completed by your chapter.

Chapter Education Seminar Completion Form – use to record completion of the chapter education requirement. Must be submitted to the PHC or IFC VP of Risk Management’s box in 102 Sproul prior to the last day of classes.

IFC Security Reimbursement Request – use to request a reimbursement (25% for social events, 50% for philanthropy events) for security hired for an event held by your chapter.

Chapter Education Seminar Completion Form – print and submit this form to 102 Sproul after completion of a required chapter education seminar from a qualified professional.

Sample Summer Housing Lease – use this lease for summer boarders in order to ensure that they are held responsible for damages, rent due, etc. while living in your chapter house.
